Candidate Recommendation Form for Board Membership
The Meadows Mental Health Policy Institute (Meadows Institute) works to help more people get effective mental healthcare when and where they need it. From prevention and early intervention to treatment and crisis response, we partner with government agencies, health systems, and community organizations to strengthen systems that deliver on a simple promise: care that works for the people who need it.
Board members play a central role in advancing that mission by bringing credibility, networks, judgment, and influence to the Meadows Institute’s work. As you consider a nomination, we ask that you recommend only those whom you believe have the character, commitment, and lived or professional connection to mental health needed to serve in that role.
This form will help the Nomination Committee understand how the candidate could strengthen the Meadows Institute’s work and extend its reach. Please be as specific and candid as possible.
